STEP 2:

By digitally signing and submitting our enrollment form, we certify that we are
a K-12 school or religious organization and meet all requirements of 501(c) organizations
listed below.
- Organization must be organized and operated exclusively for religious, educational,
scientific or other charitable purposes,
- Net earnings may not inure to the benefit
of any private individual or shareholder,
- No substantial part of its activity
may be attempting to influence legislation,
- The organization may not intervene
in political campaigns, and
- No part of the organization’s purposes or activities
may be illegal or violate fundamental public policy.

40 characters maximum. This is how your organization name will appear on our web
site and any check payments. For example, 'ABC High School Cheerleading'

required. If you do not have an affiliation, then repeat the name of the organization in this field.

This is a 9 digit number (include any zeroes) issued by the federal government to identify businesses,
similar to a social security number for individuals. Your accountant would be familiar
with this, as it is used for any financial filings. You can also call 1-800-829-1040,
pressing 3 for Business, Dept. of Treasury IRS, Specialty Tax line. They can look
up your organization or assist you in applying for a number.
